Division of Home & Community Based Services (HCBS)

Home and Community Based Services provide support to seniors, adults with intellectual disabilities, adults with physical disabilities, individuals who are experiencing temporary limitations and any individual who needs assistance carrying out tasks in their homes. Home and Community Based Services (HCBS) are provided through Attentive Home Care, a division of Multicultural Community Family Services.

HCBS services include but are not limited to assistance in the following areas:

• Preparing light meals
• Learning to cook safely
• Traveling on public transportation
• Learning to do laundry
• Light housekeeping
• Practicing street safety
• Engaging in activities in the community (walking, community events, interaction with neighbors)

Attentive Home Care is licensed by the Pa Department of Health and is approved by the Pa Department of Public Welfare to provide Waiver Services through the office of Long Term Living and the Office of Developmental Programs. MCFS currently provide services to residents of Chester, Bucks, Delaware, Montgomery and Philadelphia Counties.
